The Health and Safety Authority (HSA) and An Garda Síochána have launched statutory investigations following the death of a 44-year-old woman in a workplace incident in Kilmeedy, County Limerick, on Friday, September 18.

The victim, named locally as Elizabeth "Liz" Lynch, a mother of three from the Newcastle West area, died when a trench being dug for a septic tank pipe collapsed on top of her. Emergency services responded to the scene, but despite the efforts of individuals present, Ms Lynch was pronounced dead.

A Garda spokesperson confirmed that officers attended the scene of a fatal workplace incident in Kilmeedy on Friday and subsequently referred the matter to the state safety watchdog. A spokesperson for the Health and Safety Authority confirmed the agency is aware of the tragedy and has initiated a formal investigation.

The accident has prompted widespread grief across the rural community. Local Fine Gael councillor Adam Teskey described the death as a devastating blow to the locality, offering condolences to the family during a time of immense mourning.

Ms Lynch was well known locally for her involvement in youth sports coaching and was a member of Monagea Ladies Gaelic Football Club. In tribute, both Monagea LGFC and Newcastle West Town FC suspended all weekend sporting activities as a mark of respect.
